StarCare Specialty Health System is seeking a full-time Juvenile Continuity of Care (CoC) Case Manager to join its outstanding TCOOMMI team (Texas Correctional Office on Offenders with Medical or Mental Impairments).
This position plays a vital role in supporting youth with severe or persistent mental health needs who are involved in the juvenile justice system. This position ensures continuity of care by coordinating mental health services, facilitating interagency collaboration, and supporting rehabilitation efforts to reduce recidivism and promote successful community reintegration.
Responsibilities:
- Provide continuity of care and case management services to juveniles referred through probation or detention who have been identified with mental health needs.
- Develop individualized service plans in collaboration with families, probation officers, and mental health providers.
- Coordinate referrals and link youth to community-based mental health, educational, and rehabilitative services.
- Monitor compliance with treatment plans and provide ongoing support to youth and families.
- Maintain accurate documentation and ensure compliance with TCOOMMI standards and contractual obligations.
- Collaborate care between StarCare Specialty Health System, Texas Juvenile Justice Department (TJJD), and other stakeholders to ensure seamless service delivery.
- Participate in multidisciplinary team meetings and provide input on service planning and discharge coordination.
Schedule: This is a full-time/non-exempt position scheduled Monday – Friday, 8am – 5pm, with flexibility to meet the needs of the department.

Qualifications:
- Graduation from an accredited four (4) year college or university with major course work in a human services field (Human Development /Family Studies, Psychology, Sociology, etc.) and one year of full-time experience in a related field. Master’s Degree Preferred with applicable licensure to include: LPC, LMFT, or an LCSW.
- Experience working with juvenile populations and/or individuals with mental health needs.
Skills and Abilities:
- Knowledge of juvenile justice systems, mental health services, and community resources in Texas.
- Strong communication, organizational, and documentation skills.
- Ability to work independently and as part of a collaborative team.
Additional Requirements:

- Must pass a pre-hire drug screen and criminal background check.
- Must have a Texas driver's license, liability auto insurance, and be insurable under the Agency's insurance.
- Must meet eligibility requirements for working with justice-involved youth.
