StarCare Specialty Health System is seeking a full-time Adult Case Manager to join its outstanding TCOOMMI team (Texas Correctional Office on Offenders with Medical or Mental Impairments).
This position provides comprehensive case management for adults with severe or persistent mental health needs who are involved in the criminal justice system. The role focuses on coordinating mental health services, maintaining close collaboration with probation and parole officers, and supporting rehabilitation efforts to reduce recidivism and promote successful reintegration into the community.
Responsibilities:

Provide case management services to adults referred through probation or parole who have been identified with mental health needs.
· Develop individualized service plans in collaboration with clients, probation/parole officers, and mental health providers.
· Coordinate referrals and link clients to community-based mental health, housing, employment, and rehabilitative services.
· Monitor compliance with treatment plans and provide ongoing support to clients.
· Maintain accurate documentation and ensure compliance with TCOOMMI standards and contractual obligations.
· Collaborate with StarCare Specialty Health System, Texas Department of Criminal Justice (TDCJ), and other stakeholders to ensure seamless service delivery.
· Participate in multidisciplinary team meetings and provide input on service planning and discharge coordination.
Schedule: This is a full-time/non-exempt position scheduled Monday – Friday, 8am – 5pm, with flexibility to meet the needs of the department.

Qualifications:
- Graduation from an accredited four (4) year college or university with major course work in a human services field (Human Development /Family Studies, Psychology, Sociology, etc.),
- Two (2) years' full-time experience in a related field, preferred.
Skills & Abilities:
- Knowledge of criminal justice systems, mental health services, and community resources in Texas.
- Strong communication, organizational, and documentation skills.
- Ability to foster collaborative relationships with key workforce members.
- Exhibit excellent oral and written communication with the ability to communicate to all levels of the organization.
- Organized, detail oriented, computer proficient, and have the ability to write clearly and concisely.
- Ability to respond to individuals with patience, empathy, and understanding.
- Experience in working with individuals with serious mental illness, substance use disorders, and/or severe emotional disturbances.
- Knowledge of the social services system and the ability to guide workforce members in connecting individuals with appropriate services.
- Ability to work under general supervision with moderate latitude for the use of initiative and independent judgment.
- Knowledge and skill in operating standard office equipment (phone, fax, copier, printer) and personal computer with Microsoft Office products is required.
Additional Requirements:
- Must meet TDCJ/TCOOMMI eligibility requirements for working with justice-involved individuals.
- Must pass a pre-hire drug screen and criminal background check.
- Must have a Texas driver's license, auto liability insurance, and be insurable under the Agency's insurance.
