Triage Case Manager Responsibilities:
- Serve as the initial point of intake, screening and referral for all individuals seeking Intellectual Developmental Disabilities, mental health, and /or Substance Use Disorder Services.
- Provide telephone or face-to-face screenings to determine if caller meets state designated definitions of priority population.
- Complete CANS (Children’s Needs and Strengths Assessment) and/or ANSAs (Adult Needs and Strengths Assessment) as needed to assist with determination of levels of care.
- Follow up with individuals by phone call related to appointments and scheduling.
- Complete initial mental health and substance use screenings for callers from the 5 (five) county catchment area (Lubbock Lynn, Crosby, Cochran and Hockley) and OSAR’s (Outreach, Screening, Assessment, & Referral) catchment area, Region 1, that includes 41 counties.
- Schedule all assessments and OSAR appointments utilizing StarCare’s electronic health record.
- Make appropriate referrals.
- Attend all required meetings and trainings.
- Communicate appropriate information to supervisor and other StarCare staff.
- Assist supervisor and other StarCare staff with special assignments and/or projects.
- This position ensures all activities support the mission, values and goals of StarCare Specialty Health System.
Schedule: This is an office-based, full-time (non-exempt) position scheduled Monday – Friday, 8am – 5pm with flexibility to meet the needs of the department.

Educational Requirements:
- Bachelor’s degree from an accredited four (4) year college or university with a major in a human services field or related field (Psychology, Sociology, Family Studies, etc.) required. Experience in a related field, preferred.
Required Skills & Abilities:
- This position requires strong verbal communication skills and documentation skills, as well as the ability to effectively communicate in writing.
- Must possess excellent customer services skills, including being compassionate, patient, and kind when interacting with consumers seeking services.
- Ability to organize and prioritize a variety of assignments and manage time effectively.
- Ability to maintain effective working relationships with individuals seeking and receiving services, other employees, and the public.
- Knowledge and skills in operating personal laptop and Microsoft Office products and standard office equipment (phone, fax machine, copier, etc.).
Preferred Skills & Abilities:
- Knowledge of human service delivery systems.
- Bilingual (English/Spanish).
Additional Requirements:
- Must pass a pre-hire drug screen and criminal background check.
- Must earn and maintain ANSA and the CANS certification, post hire.
- Must have a Texas driver's license, liability auto insurance, and be insurable under the Agency's insurance.
- Must be able to sit in an office setting and speak on the telephone for extended periods of time.
