StarCare Specialty Health System is in search of a well-organized and dynamic professional to join the Local Intellectual and Developmental Disability Authority (LIDDA) as the Habilitation Coordinator to fill an essential role in the Pre-admission Screening and Resident Review (PASRR) process. This position provides assistance for a person living in a Nursing Facility to access appropriate specialized services necessary to achieve a quality of life and level of community participation acceptable to the person and LAR on the person’s behalf.

 

 Responsibilities of PASRR Habilitation Coordinators Include:

  • Using the discovery process to develop the Individual Plan (IP) and Habilitation Service Plan (HSP).
  • Assess or reassess habilitative needs.
  • Ensure all Specialized Services are implemented in a timely manner.
  • Conduct monthly face-to-face visits with individuals in nursing facilities.
  • Conduct semi-annual Community Living Options (CLO).
  • Perform on-going monitoring activities.
  • Be responsible for scheduling and facilitating the service planning team (SPT) meetings at least quarterly and revise HSP as needed.
  • Assist with completing PASRR Evaluations (PE) when needed and when time permits.
  • Communicate information to supervisor and other employees as needed. 

 

Schedule: 

Full-time (40 hours per week) non-exempt position scheduled Monday – Friday, 8am – 5pm with flexibility to meet the needs of the program.

Qualifications:

  • Bachelor’s or advanced degree from an accredited four (4) year college or university with major in a Human Services or related field (Psychology, Social Work, Human Development, etc.), and one (1) year of full-time experience working with individuals with Intellectual or Developmental Disabilities (IDD) is required.
  • Experience working with individuals with Mental Illness is preferred.

 

Additional Qualifications:

  • Experience with Pre-Admission Screening and Resident Review (PASRR) is preferred.
  • Bilingual (English/Spanish) also preferred.
  • Experience working with Nursing Facilities, preferred.
  • Knowledge and skill in operating a personal computer with Microsoft Office products is required.
  • Must be able to communicate effectively, both orally and in writing.
  • Must be able to organize and prioritize a variety of assignments and manage time effectively.
  • Must be able maintain effective working relationships with clients, other employees, and the public.

 

Additional Requirements:

  • Must pass a pre-hire drug screen and criminal background check.
  • Must have a Texas driver's license, liability auto insurance, and be insurable under the Agency's insurance.
