StarCare is in search of a qualified and compassionate professional to join the Assertive Community Treatment (ACT) Team.
We are seeking a compassionate, dedicated, and collaborative Case Manager to join our Assertive Community Treatment (ACT) team. As an ACT Team Member, you will provide comprehensive, community-based psychiatric treatment, rehabilitation, and support services to individuals with serious mental illness. This role is ideal for someone who thrives in a multidisciplinary environment and is passionate about making a meaningful impact in the lives of others.
Key Responsibilities:
- Deliver direct services including case management, counseling, crisis intervention, and medication support.
- Participate in team meetings to coordinate care and develop individualized treatment plans.
- Conduct outreach and home visits to engage clients in their natural environments.
- Collaborate with psychiatrists, nurses, social workers, and peer specialists to ensure holistic care.
- Monitor client progress and adjust interventions as needed.
- Maintain accurate and timely documentation in compliance with agency and regulatory standards.
- Provide 24-hour on-call coverage as part of a rotating schedule.
HOURS: 8:00 am – 5:00 pm, Monday through Friday, with flexibility to meet customer & program needs.

Job Qualifications
Qualifications:
- Graduation from an accredited four (4) year college or university with major course work in a human services field (Human Development /Family Studies, Psychology, Sociology, etc.).
- Two (2) years full-time experience in a related field.
Skills & Abilities:
- Familiarity with evidence-based practices such as Motivational Interviewing, Trauma-Informed Care, and Harm Reduction.
- Knowledge and skill in operating standard office equipment (phone, fax, copier, printer) and use of Microsoft Office products is required.
Additional Requirements:
- Work laptop and cell phone will be issued.
- Must have current Texas driver's license, liability auto insurance, and be insurable under Center insurance.
- Must pass a criminal background check and drug screen.
