StarCare Specialty Health System (StarCare) is seeking a highly skilled, fully licensed mental health professional to serve as a Transition Support Team Lead (TSTL) within our Money Follows the Person (MFP) Initiative. This role is ideal for clinicians who are passionate about providing educational opportunities, interagency collaboration, and improving outcomes for individuals with intellectual and developmental disabilities who experience complex mental health and behavioral needs.
This position offers the opportunity to lead statewide-impact work, support community providers, and influence diversion from institutional settings—while working within a supportive, mission-driven organization.
Work is performed under the general supervision of the Program LIDDA Director, the Transition Support Team Lead operates with significant independence, professional discretion, and leadership authority.
Responsibilities:
- Provide advanced support and consultation to community service providers serving individuals with IDD who are:

- At risk of institutionalization, or
- Transitioning from State Supported Living Centers (SSLCs), Nursing Facilities (NFs), or medium/large ICF-IID settings
- Develop comprehensive Transition Support Team (TST) plans for submission and approval by the Health and Human Services Commission (HHSC)
- Deliver educational and training opportunities (webinars, conferences, targeted correspondence) to enhance provider and LIDDA expertise
- Offer technical assistance on complex medical, behavioral, psychiatric, and co-occurring conditions using evidence-based best practices
- Participate in case-specific peer review and multidisciplinary consultations to strengthen service planning and outcomes
- Establish and facilitate a stakeholder committee to guide program effectiveness and address system needs
- Collect, analyze, and report quarterly program data to support evaluation and improvement
Essential Duties:
- Coordinate the multidisciplinary Transition Support Team and oversee technical assistance activities
- Collaborate closely with:
- Assistant District Attorney and Mental Health Courts
- IDD Crisis Intervention Specialists
- Compile and complete Multidisciplinary Team (MDT) reports for court proceeding
Schedule: This is a full-time/exempt position scheduled Monday – Friday, 8am – 5pm, with flexibility to meet the needs of the department.

Qualifications:
- Master’s degree from an accredited university in a Human Services field (Psychology, Sociology, Family Studies, or related discipline)
- Fully Licensed Health-Related Professional, such as:
- Qualified Intellectual Disability Professional (QIDP) as defined under 42 CFR §483.430(a)
- Minimum of two (2) years of experience working with individuals with IDD who have mental health and behavioral support needs, or in coordinating mental health services for individuals with IDD
- Bilingual (Spanish/English) preferred
Skills & Abilities:
- Strong written and verbal communication skills
- Experience delivering professional presentations or training is preferred
- Proficient with Microsoft Office and standard office technology
- Ability and willingness to travel using a personal vehicle (mileage reimbursed at StarCare’s established rate)
Additional Requirements:
- Must pass a pre-hire drug screen and criminal background check.
- Must have a Texas driver's license, personal auto liability insurance, and be insurable under the Agency's insurance.
